Job summary

Sweetwater Care and Rehab is seeking a dedicated Physical Therapist to join our in-house therapy team. You will perform treatments, develop individualized plans, supervise assistants, and maintain comprehensive patient records to support recovery and mobility goals.

The role requires active state licensure and strong collaboration with the IDT to deliver evidence-based care and effective discharge planning.

Qualifications

• Licensed Physical Therapist in good standing in the state.
• Experience supervising PT assistants and rehab aides.
• Excellent communication with patients and care team.
• Ability to develop individualized treatment plans.

Responsibilities

• Perform treatments and exercises to help patients recover.
• Develop personalized treatment plans and educate patients.
• Supervise PT assistants and rehab aides.
• Maintain patient records and regulatory documentation.
• Communicate progress with supervisors and care teams.
• Provide discharge summaries per regulatory requirements.
