Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in Olivia, MN. The position offers full-time, part-time, or PRN options with a $5,000 sign-on bonus and tuition reimbursement.

You will plan, direct, and supervise OT services to ensure high-quality resident care in compliance with applicable standards. Qualified candidates hold an OT degree and an active license in the practicing state, with ongoing continuing education to maintain licensure.

Qualifications

• Degree in Occupational Therapy and active OT license in practicing state(s).
• Commitment to ongoing continuing education and licensure maintenance.
• Strong communication with residents and interdisciplinary team.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop treatment plans with goals.
• Communicate evaluation findings and plans to residents and families.
• Schedule resident therapy with team members.
• Document and bill OT services according to plan.
• Perform duties to effectively treat patients.
• Oversee COTA and ensure proper documentation and reimbursement.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
