Evolve Therapy Services, a therapy management organization, is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Olivia, MN.

Full-time with a $5,000 sign-on bonus and tuition reimbursement; Part-Time or PRN options available. Must hold OTA licensure, maintain ongoing CEU requirements, and enjoy competitive pay and benefits.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular resident screenings to determine best intervention per scope of practice.
• Implement treatment plans and follow short- and long-term goals based on the plan of care.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Establish resident therapy schedules in cooperation with the team.
• Deliver therapy per established plan and ensure proper documentation and billing of occupational therapy services.
• Perform additional duties as assigned to treat patients.
• Adhere to CMS documentation standards daily with point-of-care documentation.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time ($5,000 sign on + Tuition Reimbursement!), Part Time or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice

• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy

• MUST have active OTA licensure in state(s) of practice

• Ongoing continuing education and professional development to maintain current licensure and certification
