Job summary

Collage Home and Community Rehabilitation Partners is seeking experienced Occupational Therapists to deliver patient-centered neurological rehabilitation in San Gabriel, CA and surrounding areas. PRN positions are available, with services conducted in patients' homes and communities.

Therapists work within interdisciplinary teams to implement integrated, evidence-based treatment plans tailored to functional goals.

Qualifications

• Completion of Bachelor27s degree in Occupational Therapy and state licensure is required.
• Professional certification/Master27s degree in Occupational Therapy is preferred.
• Experience in treating neurologically related disorders is strongly preferred.
• Excellent planning, organization, writing and time management skills are required.

Responsibilities

• Evaluation and treatment of impairments including upper extremity function, ADLs and IADLs, visual and perceptual skills, cognitive needs, adaptive equipment needs, and assistive technology needs.
• Provide truly patient-centered care in patients27 homes and communities while collaborating with interdisciplinary teams.

We are looking for experienced Occupational Therapists who want to provide truly patient-centered care in San Gabriel, CA and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
