Job summary

Elara Caring is seeking an Occupational Therapist to provide high-quality home-based therapy across our service area. You will evaluate patients, develop and implement plans of care, perform re-evaluations, and coordinate with physicians and the care team to ensure seamless discharge planning.

The role emphasizes teaching activities of daily living, maintaining documentation, and supervising clinical staff while traveling up to 50% within the branch/office coverage area.

Qualifications

• Graduate of an accredited OT program.
• Current, unrestricted OT license for the state of work.
• Minimum one year of clinical care experience.
• Willing to travel 50% and have reliable vehicle.
• Ability to frequently lift up to 50 pounds.

Responsibilities

• Evaluate patients and determine rehabilitation needs; develop a Plan of Care.
• Implement and adjust the Plan of Care for quality and discharge planning.
• Perform initial and re-evaluations during visits.
• Communicate significant findings to supervisor, physician and care team.
• Instruct patients and families in activities of daily living and home program.
• Supervise COTA and other clinical staff; ensure policy adherence.
