Job summary

Elara Caring is seeking an Occupational Therapist to evaluate patients in home settings and develop comprehensive care plans. You will implement and adjust plans, conduct initial and follow-up visits, and ensure timely discharge planning across our care teams.

The role requires travel up to 50% within the branch/office coverage area, with focus on high-quality, patient-centered home care delivery in the community.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one year of experience in a clinical care setting.
• Able and willing to travel 50% within branch/office coverage.

Responsibilities

• Evaluates the OT patient to determine rehabilitation needs and develops a Plan of Care.
• Implements and develops the Plan of Care for quality and discharge planning.
• Initial therapy evaluation visit and re-evaluations during each visit.
• Follow-up assessments per regulatory guidelines (OASIS, PPS).
• Communicates findings to supervisor, physician, and care team.
• Initiates safety plan using patient, family, and community resources.
• Writes orders and documents therapy care per policy.
• Supervises COTA/HHA/clinical staff to ensure plan is followed.
