Qualifications

• Active/Valid Illinois license in Occupational Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Develop personalized treatment plans using a variety of techniques to maximize safety and performance in ADL and IADL.
• Ensure all treatment delivered aligns with the established plan of care.
• Monitor patient response to treatment and modify approach with IDT collaboration.
• Ensure documentation is thorough and timely and compliant with regulatory requirements.
