Nexushm in Illinois is seeking an Occupational Therapist to evaluate and treat residents, helping them regain, maintain, or improve daily living skills and functional independence.

You will develop personalized treatment plans using a variety of techniques to maximize safety and performance in ADL/IADL, monitor responses, and modify approaches as needed with IDT collaboration. Documentation must meet regulatory requirements and company standards.

Qualifications

• Active Illinois license in Occupational Therapy required.
• Experience in SNF/Long-term care preferred.
• Current knowledge of treatment practices and documentation standards.

Responsibilities

• Develop personalized treatment plans to maximize safety and independence in ADL/IADL.
• Ensure all treatment aligns with the established plan of care.
• Monitor patient response and adjust interventions with IDT collaboration.
• Complete documentation thoroughly and in compliance with state/federal requirements.
