Job summary

Nexushm is seeking an Occupational Therapist to evaluate and treat residents to regain independence in ADL/IADL in Illinois facilities. Missouri-licensed therapists are welcome, and the company will cover the cost of Illinois reciprocity licensing.

Responsibilities include developing personalized treatment plans, ensuring plans of care are followed, monitoring responses, and documenting thoroughly per regulatory standards. Strong knowledge of PCC documentation is preferred.

Qualifications

• Active Illinois license in Occupational Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of PCC Electronic Documentation System preferred.

Responsibilities

• Developing personalized treatment plans using a variety of treatment techniques to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL).
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
