LifeSpan Therapy is seeking an Occupational Therapist to support residents in Brown County. The role focuses on evaluating functional abilities, developing personalized treatment plans, and collaborating with a multidisciplinary team to enhance independence and quality of life.

Qualifications

• Licensed therapist with ACOTE-accredited program.
• Indiana driver's license and ability to travel to other sites.
• Experience in resident care, assessment and treatment planning.
• Basic computer literacy for documentation.

Responsibilities

• Conduct thorough resident evaluations to determine functional abilities and limitations.
• Develop and implement individualized treatment plans based on assessments and goals.
• Provide interventions to improve residents' motor skills, cognitive functions, and daily living activities.
• Assign residents to a COTA when appropriate and communicate the plan to the care team.
• Monitor resident progress and adjust treatment plans accordingly.
• Educate residents, families, and caregivers on techniques and strategies for rehabilitation.
• Maintain accurate documentation of care, plans, and progress notes.
• Collaborate with healthcare professionals to coordinate care and optimize recovery.
• Adhere to infection control and maintain a safe therapeutic environment.
