Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house skilled nursing facility in Olivia, MN to plan, organize, develop and direct OT services in line with federal, state, and local standards.

You will evaluate residents, develop treatment plans and goals, coordinate with the interdisciplinary team, supervise COTA as needed, and ensure accurate documentation and compliant reimbursement practices.

Qualifications

• Degree in Occupational Therapy required.
• Active OT license in Minnesota required.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best interventions and treatments.
• Evaluate residents to inform treatment plans and goals.
• Communicate evaluation findings to residents, families, and team.
• Establish resident therapy schedules with team.
• Perform therapy and ensure proper documentation and billing.
• Oversee COTA and ensure compliance with reimbursement practices.
