Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility located in Oklahoma City, OK. The role supports comprehensive OT services under federal and state standards to ensure high-quality resident care.

Positions available include Full Time, Part Time, or PRN. Responsibilities include screening, evaluation, planning, scheduling, documentation, and supervision of COTAs, with opportunities for growth within the organization.

Qualifications

• Must have an Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments follow clinical excellence and proper reimbursement practices.
