Job summary

Evolve Therapy Services in Oklahoma City, OK seeks an Occupational Therapist to plan, organize, develop, and direct occupational therapy services in a Skilled Nursing Facility, ensuring care meets federal, state, and local standards.

The role requires therapeutic screening, evaluation, treatment planning, scheduling, documentation, and coordination with the interdisciplinary team, with supervision of COTA as needed. Active OT license and ongoing continuing education are essential.

Qualifications

• Must have an active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy is required.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed.

Education

Degree in Occupational Therapy

Active Occupational Therapy license

Continuing education to maintain licensure
