Job summary

Evolve Therapy Services, LLC is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in New Jersey. The role requires planning, directing OT services and coordinating with an interdisciplinary team to ensure high-quality resident care.

The OT will screen and evaluate residents, develop treatment plans, and manage therapy schedules while maintaining accurate documentation and billing. Active licensure and ongoing professional development are required.

Qualifications

• Active OT license required in the state(s) of practice.
• Degree in Occupational Therapy from an approved program.
• Commitment to ongoing continuing education and professional development.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and set short- and long-term goals.
• Communicate evaluation findings and treatment plans to residents and families.
• Establish individual therapy schedules with the team.
• Provide OT therapies according to the plan and ensure proper documentation and billing.
• Perform other duties as assigned to support patient care.
• Oversee COTA and ensure documentation and treatments meet standards.
