Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in New Jersey. The role focuses on planning, directing occupational therapy services in accordance with federal, state, and local standards to ensure high-quality resident care.

Responsibilities include screening, evaluating, planning, coordinating schedules, and supervising a COTA. Active state license and ongoing CE are required; full-time or PRN positions available.

Qualifications

• Must hold an active Occupational Therapy license in practicing state.
• Degree in Occupational Therapy required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings and plans to residents, families, and team.
• Schedule resident therapy in collaboration with the team.
• Deliver therapy per plan and document and bill services.
• Oversee COTA and ensure documentation and reimbursement practices are followed.
• Perform other duties as needed to treat patients.
