Job summary

Evolve Therapy Services seeks an Occupational Therapist for an in-house Skilled Nursing Facility in New Jersey. You will plan, direct OT services, and ensure quality resident care in compliance with standards, regulations, and facility direction.

Responsibilities include screening residents, developing treatment plans with goals, communicating results to families and the team, scheduling therapy, documenting and billing, and supervising COTAs to maintain clinical excellence.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.
• OT degree required.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings and plans to residents, families and the interdisciplinary team.
• Establish resident therapy schedules with other team members.
• Deliver therapy according to the treatment plan and document/bill OT services.
• Oversee COTA and ensure documentation and reimbursement practices are maintained.
