Job summary

Evolve Therapy Services is seeking an Occupational Therapist to join an in-house Skilled Nursing Facility in West Orange, NJ. The role supports plan development, therapy scheduling, documentation, and collaboration with the care team to deliver high-quality resident care.

Full-time, part-time, or PRN positions are available. Active OT license in practicing state(s) and ongoing continuing education are required to maintain licensure and provide exceptional clinical services.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine intervention or treatment.
• Evaluate residents to inform treatment plans and set goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations.
• Establish individual resident therapy schedules with the team.
• Deliver therapy per plan and ensure proper documentation and billing.
• Perform other duties as required to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments meet standards.

Education

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN positions available

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
