Evolve Therapy Services, LLC in West Orange, NJ is hiring an Occupational Therapist for an in-house Skilled Nursing Facility. The role focuses on planning, directing OT services and collaborating with a multidisciplinary team to maintain high-quality resident care.

Full-time, part-time, or PRN positions available. Requires OT degree and active license in New Jersey, with ongoing continuing education.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents to inform and develop treatment plans with short- and long-term goals.
• Communicate evaluation findings and treatment plans to residents, families, and the interdisciplinary team.
• Create individual OT schedules in coordination with the care team.
• Deliver therapy per plan and document care for billing.
• Perform additional duties as required to effectively treat patients.
• Supervise COTA as needed and ensure proper documentation and reimbursement practices.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
