Job summary

Evolve Therapy Services, LLC in City of Orange, NJ seeks an Occupational Therapist for an in-house Skilled Nursing Facility. You will plan, organize, and direct OT services to maintain high-quality resident care in accordance with applicable standards.

Full-time or PRN position with possible supervision of COTA. Responsibilities include screenings, assessments, treatment planning, scheduling, documentation, and coordination with families and interdisciplinary teams.

Qualifications

• Degree in Occupational Therapy required.
• Active OT license in the practicing state(s) required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screenings to determine the best intervention for each patient.
• Evaluate residents to inform and develop treatment plans with goals.
• Interpret and communicate evaluation findings to residents, families, and the team.
• Establish resident therapy schedules in collaboration with team members.
• Deliver therapy per treatment plan and ensure documentation and billing.
• Perform additional duties to effectively treat patients.
• Oversee COTA and ensure proper documentation and reimbursement practices.
