Job summary

Atmore Community HomeCare, part of LHC Group within the Optum family, is seeking an Occupational Therapist for home health to assess and evaluate patient care needs related to functional status, activities of daily living, and adaptive equipment.

You will work within the plan of care, coordinate with the care team, document CMS-relevant status, and support patients across diverse home settings. A current OT license and a valid driver's license are required, with CPR preferred.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle, or public transportation.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team
• Evaluates the patient's functional status, CMS documentation requirements, and occupational therapy needs; consults with the physician in the development of the plan of care
• Observes, records, and reports to the nurse supervisor and physician the patient's response to treatment and changes to condition
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
