Job summary

Atmore Community HomeCare, part of the LHC Group and Optum family, seeks an Occupational Therapist in Home Health to assess and address patients’ functional status, activities of daily living, and adaptive equipment in home settings.

You will evaluate, plan, and coordinate care with physicians, document responses, and perform assessments to support CMS requirements. Qualified candidates hold OT licensure and a valid driver’s license; CPR is preferred.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to dependable vehicle, or public transportation.
• CPR certification (preferred) or ability to obtain within 90 days of hire.
• Ability to manage multiple tasks simultaneously and work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy and in accordance with the plan of care.
• Evaluates the patient’s functional status and therapy needs and consults with the physician in developing the therapy plan of care.
• Observes, records, and reports patient response to treatment to the nurse supervisor and physician.
• Conducts patient assessments evaluating functional level using diagnostic and prognostic tests.
• Supports ability to function in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
