Job summary

Atmore Community HomeCare, part of the LHC Group within the Optum family, seeks an Occupational Therapist in Home Health to assess and evaluate patient care needs, ADLs, and home environments. You’ll work with the care team to develop and implement plans of care and monitor progress.

Qualifications include current OT licensure in the practicing state and reliable transportation. Preferred: CPR certification within 90 days, ability to manage multiple tasks, and strong communication skills.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Reliable transportation or access to a vehicle.

Responsibilities

• Evaluates patient function and develops therapy plan of care with physician input.
• Assesses CMS documentation needs and coordinates with the health care team.
• Observes and reports patient responses to treatment to supervision and physician.
• Conducts functional ability assessments using standard tests.
• Provides care in diverse home environments and respects patient dignity.
