Optum's Atmore Community HomeCare, part of LHC Group, seeks an Occupational Therapist to assess and plan care for home health patients, focusing on functional status, activities of daily living, and adaptive equipment.

You will collaborate with a multi-disciplinary team, document CMS-required status, and support patient independence across diverse home settings, ensuring quality care and compliance with regulations.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsabilités

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
• Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs; consults with the physician in developing the plan of care.
• Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
