Job summary

Atmore Community HomeCare, part of the LHC Group, seeks an Occupational Therapist to provide care in home health settings with a focus on evidence-based practice and CMS documentation. You will work within the plan of care, coordinate with the health care team, and assess functional status to guide therapy decisions.

The role requires state licensure, a valid driver’s license, and the ability to work in diverse home environments.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
• Evaluates the patient’s functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care
• Observes, records, and reports to the nurse supervisor and the physician the patient’s response to treatment and changes to the patient's condition
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
