Job summary

All Seniors Foundation is seeking a Home Health Occupational Therapist (OT) to help seniors perform daily tasks with greater independence in their own homes. You will create personalized plans, apply adaptive techniques, and guide families through rehabilitation strategies.

Daily home visits, development of rehabilitation programs, and caregiver training are central to the role, with progress documentation and ongoing assessment to maximize client independence.

Qualifications

• California OT license required at time of application.
• Experience with seniors/home health is preferred.
• Strong communication, creativity, and problem-solving skills.

Role Overview

we seek a Home Health Occupational Therapist (OT) who can create personalized plans to help seniors perform daily tasks with greater ease and confidence. Your expertise in adaptive techniques, environmental modifications, and functional exercises will enable clients to remain independent in their own homes.

Key Responsibilities

• Assess clients’ functional abilities and challenges within their home environment.
• Develop tailored intervention plans to improve daily living skills.
• Recommend adaptive equipment, home modifications, and ergonomic adjustments.

• Valid Occupational Therapist license in California.
