Job summary

All Seniors Foundation is seeking an Aging-in-Place Consultant to lead home safety assessments and plan modifications that help seniors stay safely at home in Los Angeles County. You will identify fall risks, recommend equipment like grab bars and ramps, and coordinate installation with contractors and suppliers.

Qualified applicants will be licensed OTs in California (OTR/L); CAPS certification is preferred.

Qualifications

• Licensed Occupational Therapist with CA OTR/L
• CAPS certification preferred
• 2+ years OT experience, home health or aging-in-place focus a plus
• Familiarity with ADA, AOTA, and CA accessibility codes
• Strong assessment, communication, and project-management skills

Responsibilities

• Conduct comprehensive home safety assessments using the AOTA Home Modification framework
• Identify fall hazards, accessibility barriers, and adaptive-equipment needs
• Recommend modifications: grab bars, ramps, raised toilet seats, stair-glides, smart-home tech
• Coordinate with contractors, CAPS specialists, and DME vendors on installation
• Educate seniors and families on energy conservation, fall prevention, and home safety
• Document assessments and follow up on completed modifications

Role Overview

As Aging-in-Place Consultant, you will lead home assessments that allow seniors to age safely in their own homes. You will identify fall risks, recommend modifications (grab bars, ramps, lighting), and coordinate installation with contractors and equipment vendors.

Key Responsibilities

• Conduct comprehensive home safety assessments using AOTA Home Modification framework
• Identify fall hazards, accessibility barriers, and adaptive-equipment needs
• Recommend modifications: grab bars, ramps, raised toilet seats, stair-glides, smart-home tech
• Coordinate with contractors, CAPS specialists, and DME vendors on installation
• Educate seniors and families on energy conservation, fall prevention, and home safety
