Talin Home Healthcare in Virginia is seeking an experienced Occupational Therapist to join our in-home care team. You will assess patients, design personalized therapy plans, and apply interventions to improve daily living activities.

You will collaborate with other clinicians, educate families, and document progress while traveling to Fairfax City, Arlington, and surrounding areas. A current OT license and accredited degree are required; mileage reimbursement is available.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Active state OT licensure.
• Strong knowledge of physiology and occupational health principles.
• Experience in case management and working with individuals with disabilities preferred.
• Excellent communication with patients and teams.
• Compassionate with commitment to rehab outcomes.

Responsabilités

• Conduct comprehensive evaluations of patients' needs.
• Develop and implement individualized care plans.
• Provide direct patient care to improve daily living skills and overall functionality.
• Collaborate with speech therapists, case managers and others for holistic care.

Duties

• Conduct comprehensive evaluations to assess patients' physical, emotional, and social needs.
• Develop and implement individualized care plans tailored to each patient's goals and capabilities.
• Provide direct patient care through therapeutic activities designed to improve daily living skills and overall functionality.
• Collaborate with other healthcare professionals, including speech therapists and case managers, to ensure a holistic approach to patient care.
• Educate patients and their families about treatment options, progress, and strategies for ongoing support.
• Monitor patient progress and adjust treatment plans as necessary based on observed outcomes.
• Maintain accurate documentation of patient assessments, treatment plans, and progress notes in compliance with policies.

Requirements

• Degree in Occupational Therapy from an accredited program.
• Valid state licensure as an Occupational Therapist.
• Strong knowledge of physiology and occupational health principles.
• Experience in case management and working with individuals with disabilities is preferred.
• Familiarity with early intervention strategies is a plus.
• Excellent communication skills to effectively interact with patients, families, and interdisciplinary teams.
• Ability to develop effective care plans that address individual patient needs.
