Talin Home HealthCare LLC is seeking a dedicated Occupational Therapist to provide high-quality home-based patient care across Fairfax City, Fairfax County, Arlington, Alexandria, Loudoun, and surrounding areas. You will develop personalized care plans, perform assessments, and implement therapies to improve daily living skills and overall functionality.

The role involves delivering direct patient care on the road, collaborating with interdisciplinary teams, and educating patients and families

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Valid state licensure as an Occupational Therapist.
• Strong knowledge of physiology and occupational health principles.
• Experience in case management and working with individuals with disabilities is preferred.
• Familiarity with early intervention strategies is a plus.

Responsibilities

• Conduct comprehensive evaluations to assess patients' physical, emotional, and social needs.
• Develop and implement individualized care plans tailored to each patient's goals and capabilities.
• Provide direct patient care through therapeutic activities designed to improve daily living skills and overall functionality.
• Collaborate with other healthcare professionals, including speech therapists and case managers, to ensure a holistic approach to patient care.
• Educate patients and their families about treatment options, progress, and strategies for ongoing support.
• Monitor patient progress and adjust treatment plans as necessary based on observed outcomes.
• Maintain accurate documentation of patient assessments, treatment plans, and progress notes in compliance with policies.

Job Overview

Talin is seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will be responsible for providing high-quality patient care in a patient's homes, focusing on improving the functional abilities of individuals with disabilities, injuries, recovery and aging. This role involves developing personalized care plans, conducting assessments, and implementing therapeutic interventions to enhance patients' quality of life.

Duties
