Job summary

Moorings Park in Naples, FL seeks a dedicated Occupational Therapist to join our Certified Home Health team. The OT coordinates quality, safe home health services in compliance with licensure and plan of care, with supervision by a licensed OT.

Responsibilities include evaluating, planning, and delivering OT interventions, supervising staff, and coordinating with physicians and the Clinical Manager to promote resident independence and well-being.

Qualifications

• Excellent oral and written communication in English.
• Florida license to practice occupational therapy required.
• 2+ years of geriatric/home health experience preferred.
• Must have a valid Florida driver’s license and insurance.

Responsibilities

• Evaluate patients to determine OT care needs and plan.
• Develop and implement individualized Plan of Care with goals.
• Provide OT treatments and ADL retraining per plan.
• Supervise OT Assistant and Home Health Aides.
• Complete OASIS assessments and related documentation.
