Job summary

Moorings Park in Naples, Florida, is seeking a dedicated Physical Therapist to join our team. The role focuses on delivering high-quality home health physical therapy services, coordinating care with an interdisciplinary team, and ensuring compliance with licensure and facility policies.

The successful candidate will evaluate residents, develop and update Plans of Care, and provide hands-on therapeutic interventions while supervising PTAs and communicating progress to the physician.

Qualifications

• Graduate from an approved physical therapy program and be licensed in Florida.
• Maintain CEU and licensure requirements.
• Outpatient rehab experience preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations for SNF residents.
• Develop and update an individualized Plan of Care with the physician and team.
• Set measurable goals and monitor progress toward them.
• Provide interventions: therapeutic exercises, gait/balance training, mobility training.
• Supervise PTAs and ensure on-site supervision and documentation.
• Document evaluations, treatments, progress notes, and discharge summaries.
