Job summary

The Keystone Distinction Group is seeking an Occupational Therapist for Home Health to provide patient-centered care, assess patients, and develop individualized care plans in coordination with physicians and the care team.

Responsibilities include performing assessments, collaborating with families, and guiding home health therapy within the continuum of care to support goal achievement and positive patient experiences.

Qualifications

  • 1) Must be a graduate of an OT program approved by the AMA/AOTA.
  • 2) Must have NBCOT certification (National Certification Examination for Registered Occupational Therapist).
  • 3) State license to practice OT in the appropriate state.
  • 4) Reliable transportation and proof of valid automobile liability insurance.
  • 5) Must have a valid driver’s license.

Responsibilities

  • Assess patients and develop individualized care plans aimed at goal achievement.
  • Coordinate with physicians and the care team to ensure patient engagement and experience.
  • Provide or direct the majority of the patient’s therapy within home health and continuum of care.

OCCUPATIONAL THERAPIST - Home Health

KEY JOB RESPONSIBILITIES:

The Occupational Therapist provides high-quality, patient-centered care by managing the complete process of occupational therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team. The Occupational Therapist completes patient assessments, and participates in developing and implementing individualized care plans, focusing on goal achievement, patient engagement, and positive patient experience. The Occupational Therapist provides or directs a majority of the patient’s physical therapy to support patients throughout home health and the continuum of care.

Qualifications

PROFESSIONAL EXPERIENCE/QUALIFICATIONS:

OT Home Health Expertise

  • Must be computer literate and able maintain simple records in English.
  • Promotes patient/family education using various verbal and written communication techniques that take into account the patients/family's cultural, ethnic and/or personal needs or preferences.
  • Collaborates with the patient/family to learn appropriate self-care techniques.
  • Submits documentation in a timely manner per Agency policy.
  • Knowledgeable of current federal, state and agency regulations concerning patient care, includes incident management and reporting.

EXPERIENCE

  • Prefer at least one year of Home Health Care.
  • Comprehensive knowledge of general therapy theory and practice and able to work independently.

REQUIREMENTS:

  • 1 Year Home Health Experience
  • Bachelor of Science in Occupational Therapy from an accredited program
  • Successful completion of the National Certification Examination for Registered Occupational Therapist
  • State license in practicing state
  • Reliable transportation and proof of valid automobile liability insurance
  • Must have valid driver’s license

EDUCATION:

  • Must be a graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association.
  • Preferably should be a member of the American Occupational Therapy Association.
