Job summary

CommonSpirit Health at Home is seeking a skilled Occupational Therapist to provide in-home therapy across Parker, Aurora, and South Denver. You will assess patients, set goals, and implement personalized plans to improve independence and quality of life, while coordinating with the home health team.

The role requires an OT license in the state, CPR/BLS, and a minimum of one year of clinical experience, with a focus on safety and family education. A current driver's license is essential.

Qualifications

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association.
• Active registered OT license in the practicing state.
• CPR/BLS Certification obtained and maintained.
• At least 1 year of OT experience in a clinical setting.
• Current driver's license.
• Compliance with federal, state and local laws and regulations.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs and develop plans.
• Teach patients and families about home safety, adaptive equipment, and daily activity strategies.
• Maintain accurate records of progress, treatment plans, and modifications.
• Monitor progress and adjust plans as needed, using outcome measures to assess effectiveness.
• Collaborate and communicate patient condition changes with physicians, nurses, PTs, SLPs and home health team members.

Job Summary and Responsibilities

The primary service area for this position will be Parker, Aurora and South Denver

CommonSpirit Health at Home leverages AI-powered clinical documentation to reduce documentation, enhance work-life balance, and maximize patient time.

We are seeking a skilled and compassionate Occupational Therapis t to join CommonSpirit Health at Home. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.

Responsibilities:

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs.Develop and implement personalized treatment plans that may include exercises’, adaptive techniques, and use of assistive devices to improve patients functional abilities.

• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.

• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.

• Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.

• Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

Job Requirements

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association

• Active registered OT license in the state which you practice

• Must obtain and maintain current CPR/BLS Certification

• One-year Occupational Therapy experience in a clinical setting.

$39.27 - $64.80 /hour
