Job summary

CommonSpirit Health at Home is seeking a skilled Occupational Therapist to provide in-home therapy services in Leadville, Fairplay, and Frisco. This PRN position focuses on evaluating patient needs, setting goals, and creating plans to maximize independence and quality of life.

You will work with a multidisciplinary home health team, delivering client-centered care, and maintaining thorough documentation to ensure regulatory compliance and optimal outcomes.

Qualifications

• Bachelor’s degree from an American Occupational Therapy Association approved program.
• Active OT license in the state of practice.
• Current CPR/BLS certification.
• Minimum 1 year OT experience in a clinical setting.
• Must have a valid driver’s license.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs and develop personalized plans.
• Implement treatment plans with exercises, adaptive techniques, and assistive devices to improve independence.
• Educate patients and families on home safety and daily activity strategies.
• Maintain accurate records of progress, plans, and necessary plan modifications.
• Collaborate with physicians, nurses, and other therapists to adjust care as needs change.

We are seeking a skilled and compassionate *Occupational Therapis*t to join CommonSpirit Health at Home. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.

*Responsibilities*

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises’, adaptive techniques, and use of assistive devices to improve patients functional abilities.
• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
• Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
• Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

*Job Requirements*

• Bachelor’s degree from an accredited school approved by the American Occupational Therapy Association
• Active registered OT license in the state which you practice
• Must obtain and maintain current CPR/BLS Certification
• One-year Occupational Therapy experience in a clinical setting.
• Current Drivers License
• Comply with federal, state and local laws and regulations

*Pay Range*

$39.18 - $64.65 /hour
