Job summary

CommonSpirit Health at Home is seeking a skilled Occupational Therapist to join the St. Elizabeth's Health at Home team in Indiana. You will provide home-based therapy, assess abilities, set goals, and develop plans to promote independence and improve quality of life.

The role requires a Bachelor's in OT, active OT license, CPR/BLS, 1 year of OT experience, and a valid driver's license. You will collaborate with physicians and other home health staff; pay ranges $39.18–$58.28 per hour.

Qualifications

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association.
• Active registered OT license in the state in which you practice.
• Must obtain and maintain current CPR/BLS Certification.
• One-year Occupational Therapy experience in a clinical setting.
• Current Driver's License.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises, adaptive techniques, and use of assistive devices to improve patients’ functional abilities.
• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
• Monitor patients’ progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
• Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.
