Job summary

CommonSpirit Health at Home is seeking a skilled Occupational Therapist for our St. Elizabeth's Home Health team. The OT will evaluate patients at home, set goals, and develop plans to help patients gain independence and improve quality of life.

The role focuses on personalized plans, home safety instruction, and collaboration with the home health team to monitor progress and adjust the therapy program as needed.

Qualifications

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association.
• Active registered OT license in the state where you practice.
• Must obtain and maintain current CPR/BLS Certification.
• One-year Occupational Therapy experience in a clinical setting.
• Current Driver's License and ability to comply with laws and regulations.

Responsibilities

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs and develop personalized treatment plans.
• Teach patients and families about home safety, adaptive equipment, and strategies for daily activities.
• Maintain accurate progression notes, treatment plans, and modifications to ensure regulatory compliance.
• Monitor patient progress and adjust plans as needed using outcome-based measures.
• Collaborate with physicians, nurses, etc., and communicate changes in condition to the care team.

The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.

• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
• Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
• Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

$39.18 - $58.28/hour
