Job summary

St. Elizabeth's Health at Home is seeking a skilled Occupational Therapist to join our home health team.

The OT will evaluate patients in their homes, set goals and develop a plan to help them achieve independence and improve quality of life, while preventing potential health-related problems. Responsibilities include assessing physical, cognitive, and psychosocial abilities, creating personalized treatment plans, teaching home safety and adaptive techniques, and documenting progress for

  • Bachelor's degree in OT from an accredited program.
  • Active OT license to practice in the state.
  • CPR/BLS certification required.
  • 1 year OT clinical experience.
  • Current driver's license.

Responsibilities

  • Develop and implement personalized treatment plans including exercises and adaptive devices.
  • Educate patients and families on home safety and daily activities.

Job Summary and Responsibilities

We are seeking a skilled and compassionate Occupational Therapist to join the St. Elizabeth's Health at Home team. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.

Responsibilities
  • Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises’, adaptive techniques, and use of assistive devices to improve patients functional abilities.
  • Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
  • Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
  • Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
  • Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.
Job Requirements

Qualifications:

  • Bachelor's degree from an accredited school approved by the American Occupational Therapy Association
  • Active registered OT license in the state which you practice
  • Must obtain and maintain current CPR/BLS Certification
  • One-year Occupational Therapy experience in a clinical setting.
  • Current Drivers License
  • Comply with federal, state and local laws and regulations
