Job summary

Helping Hand Nurse, LLC is actively seeking Occupational Therapists to provide skilled home health services to patients in their homes.

Clinicians choose their own schedule, enjoy quick documentation, and receive ongoing support from a responsive clinical and administrative team.

The OT will assess functional status, develop and modify the plan of care, educate patients and families, and coordinate with physicians to optimize recovery in the home setting.

Qualifications

• Current unrestricted PT license.
• Minimum 1 year home care experience; 3–5 years preferred.
• Clean background and criminal record.
• CPR certification.
• Knowledge of computerized documentation and office software; experience with OASIS is a plus.

Responsibilities

• Visit patients in their homes and provide skilled home health therapy visits.
• Assess functional status, mobility, ROM, and home environment for rehab potential.
• Instruct patients and families/caregivers in use and care of therapeutic appliances.
• Report patient reaction to treatment or changes in condition to the physician.
• Direct physical therapy treatment and prioritize therapy needs.

Job description:

The occupational therapist will provide therapy services to patients according to a written physician's plan of care which will, in part, include:

• Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care, as well as evaluating the home environment.
• Assessing for muscle strength, mobility, gait, ROM--potential for rehab.
• Directing physical therapy treatment.
• Instructing patients and families/caregivers in the use and care of therapeutic appliances.
• Determining priority needs for physical therapy.
• Reporting to physician patient's reaction to treatment or changes in condition.

Responsibilities:

The Physical Therapist will be responsible for visiting patients in their homes and providing skilled home health care clinical visits, providing care for individuals based on personalized Plan of Care.

Qualifications/Requirements:

• Current unrestricted PT license
• Minimum one (1) year of home care experience preferred. Three (3) to five (5) years of clinical experience preferred.
• Clean background and criminal record
• CPR certified
• Knowledge of computerized documentation and office software required
• Preferred although not mandatory: experience with OASIS (although this can be trained)
