Job summary

Affinity Healthcare Solutions Skilled Care is seeking compassionate Occupational Therapists to join our Home Health team across Pennsylvania. You will evaluate patients at home, develop individualized treatment plans, and provide skilled OT services to improve ADLs/IADLs while promoting safety and independence.

We offer flexible scheduling, competitive compensation, and opportunities across FullTime, PartTime, PerDiem, and Contract roles.

Qualifications

• Active PA OT license required.
• Graduate of an accredited OT program.
• Home Health experience preferred but not required.
• Strong clinical assessment and treatment skills.
• Reliable transportation and valid driver's license.

Responsibilities

• Perform comprehensive occupational therapy evaluations in the home setting.
• Develop individualized treatment plans based on patient goals and clinical needs.
• Provide skilled occupational therapy services in patients' homes.
• Improve patients' ability to safely perform ADLs and IADLs.
• Recommend adaptive equipment and home modifications to enhance safety.
• Educate patients and caregivers on therapeutic techniques, fall prevention, and home safety.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals.
• Monitor patient progress and modify treatment plans as needed.
• Maintain accurate and timely clinical documentation.
• Complete patient notes and communications within required timeframes.
• Ensure compliance with agency policies and applicable regulations.
• Travel to patient homes within assigned coverage areas.

• Ensure compliance with agency policies and applicable regulations
• Travel to patient homes within assigned coverage areas

Qualifications

• Active Pennsylvania Occupational Therapist (OT) License
• Graduate of an accredited Occupational Therapy program

• Strong computer and documentation skills
• Reliable transportation and valid driver's license
