Job summary

Julian's Heart Homecare LLC seeks a Home Health Occupational Therapist to help patients build independence and safety in their homes. You will perform functional evaluations, conduct home safety assessments, and develop personalized care plans.

Requirements include a Master’s or Doctoral degree in Occupational Therapy, NBCOT, active state license, and 1–2 years of clinical experience, preferably in home health. A valid driver's license and ability to lift patients are essential.

• Functional Evaluations:Assess a patient's ability to complete daily tasks like bathing, dressing, grooming and cooking.
• Home Safety Assessments:Inspect the home for fall risk, poor lighting, or tight spaces, and suggest changes like grab bars or ramps.
• Personalized Care Plans:Create clear, measurable therapy goals that fit the patient's home environment and medical needs.
• Caregiver Education:Train family members and hone health aides on safe lifting, transfer techniques, and daily support.
• Adaptive Equipment Training:Teach patients how to use tools like shower chairs, reachers, or custom splints.
• Documentation:Keep accurate medical notes and track patient progress using agency software.

Qualifications

• Master's or Doctoral degree in Occupational Therapy from an accredited program.
• Minimum of 1 to 2 years of clinical experience; home health experience is strongly preferred.
• Active state occupational therapy license and registration with the National Board for Certification in OccupationalTherapy (NBCOT).
• Effective written and verbal communication
• Valid driver's license, reliable personal vehicle, and active auto insurance.
• US Citizen or Green Card holder
• Ability to bend, stand, walk and assist with lifting or transferring patients (often up to 50 lbs or more).
