Job summary

Apex Rehabilitation is seeking a Home Care Occupational Therapist (OTR) to provide in-home therapy, aiming to improve daily living and independence for clients in Long Island and Queens. You will tailor therapy plans and collaborate with families and healthcare professionals to deliver holistic care.

Qualifications

• OT degree from an accredited program.
• Active state licensure as an Occupational Therapist.
• Strong clinical and communication skills.
• Ability to adapt therapy plans to diverse client needs.
• Compassionate and patient-centered approach to care.
• Valid Driver’s License and/or reliable transportation.
• Valid Liability Insurance.

Responsibilities

• Conduct in-home assessments to evaluate clients' daily living needs and challenges.
• Develop and implement customized occupational therapy plans to address clients' goals.
• Provide therapeutic interventions to improve clients' ability to perform daily activities.
• Collaborate with clients, families, and healthcare professionals for holistic care.
• Educate clients and families on adaptive techniques and equipment for independence.
• Maintain accurate documentation of assessments, interventions, and progress.
