Job summary

Apex Rehabilitation is seeking a Home Care Occupational Therapist to provide patient‑centered in-home therapy, enhancing independence and quality of life. You will assess, plan, and implement customized OT services in clients' homes across Long Island and Queens.

Applicants should hold an OT degree and state licensure, with strong clinical and communication skills, and a compassionate approach. Driver's license and liability insurance are required. Equal opportunity employer.

• Degree in Occupational Therapy from an accredited program.
• State licensure as an Occupational Therapist.
• Strong clinical and communication skills.
• Ability to adapt therapy plans to diverse client needs.

Responsibilities

• Conduct in-home assessments to evaluate clients' daily living needs and challenges.

• Collaborate with clients, families, and healthcare professionals to ensure a holistic approach to care.
