Job summary

Apex Rehabilitation is seeking a Home Care Occupational Therapist to support clients in their homes across Long Island and Queens. The role emphasizes improving independence and quality of life through personalized therapy plans.

You will assess homes, design and implement therapy plans, and provide interventions to help clients perform daily activities. Collaboration with families and other professionals is essential, with emphasis on patient-centered care.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• State licensure as an Occupational Therapist.
• Strong clinical and communication skills.
• Ability to adapt therapy plans to diverse client needs.
• Compassionate and patient-centered care.
• Valid Driver’s License and/or reliable transportation.
• Valid Liability Insurance.

Responsibilities

• Conduct in-home assessments to evaluate clients' daily living needs and challenges.
• Develop and implement customized occupational therapy plans to address clients' specific goals.
• Provide therapeutic interventions to improve clients' ability to perform daily activities.
• Collaborate with clients, families, and healthcare professionals to ensure a holistic approach to care.
• Educate clients and their families on adaptive techniques and equipment for increased independence.
• Maintain accurate and detailed documentation of assessments, interventions, and progress.
