Job summary

Apex Rehabilitation is seeking a Home Care Occupational Therapist (OTR) to join our compassionate team. You will deliver in-home therapy, evaluating daily living needs and developing personalized plans to improve independence.

Part-time or per diem roles offer flexible hours, with opportunities to advance. A valid OT license and driver's license are required, and we value patient-centered care and strong communication. This role supports clients in Milford and surrounding communities.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• State licensure as an Occupational Therapist.
• Strong clinical and communication skills.
• Ability to adapt therapy plans to diverse client needs.
• Compassionate, patient-centered approach to care.
• Valid Driver’s License and/or reliable transportation.
• Valid Liability Insurance.

Responsibilities

• Conduct in-home assessments to evaluate clients' daily living needs and challenges.
• Develop and implement customized occupational therapy plans to address clients' specific goals.
• Provide therapeutic interventions to improve clients' ability to perform daily activities.
• Collaborate with clients, families, and healthcare professionals to ensure a holistic approach to care.
• Educate clients and their families on adaptive techniques and equipment for increased independence.
• Maintain accurate and detailed documentation of assessments, interventions, and progress.
