Job summary

Evolve Therapy Services in Natick, MA seeks an Occupational Therapist to plan, develop, and direct OT services in an in-house skilled nursing facility, ensuring quality resident care in compliance with federal, state, and local standards.

Responsibilities include screening residents, evaluating and developing treatment plans, coordinating with families and the interdisciplinary team, scheduling therapy, documenting and billing, supervising COTA, and maintaining licensure through ongoing CE.

Qualifications

• Degree in Occupational Therapy required.
• Active Occupational Therapy license in practicing state(s) required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules in coordination with team members.
• Perform therapy according to treatment plan and ensure proper documentation and billing of OT services.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence.

Education

Degree in Occupational Therapy

Active Occupational Therapy license
