Job summary

Roseville Care Center in Roseville, CA is seeking a compassionate Occupational Therapist to join our rehabilitation team on a full-time, day-shift basis. The OT evaluates residents, develops treatment plans, and delivers therapy to improve function and independence.

The ideal candidate collaborates with an interdisciplinary team, communicates effectively with residents and families, and ensures compliant documentation and evidence-based practice in a skilled nursing setting.

Qualifications

• Bachelor's degree in Occupational Therapy required; a master's or doctoral degree in Occupational Therapy preferred.
• Experience in a skilled nursing facility or rehabilitation setting is preferred.
• Proficiency with electronic documentation systems (Net Health, PointClickCare preferred).
• Strong communication, organizational, problem-solving, and interpersonal skills.
• Ability to work effectively with an interdisciplinary healthcare team and provide resident-centered care.

Responsibilities

• Evaluate residents to determine occupational therapy needs, functional limitations, and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Monitor resident progress and adjust treatment plans to meet goals.
• Educate residents, families, and caregivers on goals, home exercise programs, and safety techniques.
• Supervise COTAs, therapy aides, and students as applicable.
• Communicate regularly with physicians, nursing staff, and families regarding progress and discharge planning.
• Maintain timely, accurate documentation in medical records per facility policies.
