Job summary

Roseville Care Center in Roseville, CA is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide services to improve mobility, strength, balance, and independence in a skilled nursing setting.

The ideal candidate will have strong communication and collaborate with an interdisciplinary team, guiding residents toward their highest possible function and quality of life.

Qualifications

• Bachelor's degree in Physical Therapy required; advanced degree preferred.
• Experience in a skilled nursing or rehabilitation setting preferred.
• Proficiency with EMR documentation and related software (Net Health, PointClickCare) preferred.
• Strong communication and collaboration with interdisciplinary teams.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Monitor progress and adjust treatment plans to meet goals.
• Supervise PTAs, aides, and students in care activities.
• Communicate with physicians, nursing staff, residents, and families regarding progress and discharge planning.
• Complete timely documentation in medical records according to policies.
