Job summary

Roseville Care Center in Roseville, CA is seeking a full-time Occupational Therapist (OT) to join our rehabilitation team. You will evaluate residents, develop individualized plans, and provide services to enhance mobility and independence.

The ideal candidate collaborates with an interdisciplinary team, maintains compliant documentation, and guides COTAs and students. California OT license and CPR may be required; day shift schedule offered.

Qualifications

• Bachelor's degree in Occupational Therapy required; master's or doctoral preferred.
• Experience in a skilled nursing facility or rehabilitation setting preferred.
• Proficiency with electronic documentation systems; Net Health and PointClickCare preferred.
• Ability to work with an interdisciplinary healthcare team.
• Strong communication, organizational, and problem-solving skills.

Responsibilities

• Evaluate residents to determine OT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Monitor progress and adjust plans to meet goals.
• Educate residents and families on goals, home programs, safety, and ADLs.
• Recommend and facilitate durable medical equipment and adaptive devices.
• Supervise COTAs, therapy aides, and students.
• Assist nursing with Restorative Aides training.
• Communicate with physicians and families regarding progress and discharge planning.
• Participate in care conferences and rehab meetings.
• Maintain compliant documentation.
