Job summary

Roseville Care Center in Roseville, CA is seeking a compassionate Occupational Therapist to join our rehabilitation team on a full-time, day-shift basis. You will evaluate residents, develop treatment plans, and provide therapeutic services to improve functional abilities and independence.

The ideal candidate collaborates with an interdisciplinary team, communicates with residents and families, and adheres to facility policies, infection control, and documentation requirements.

Qualifications

• Bachelor’s degree in Occupational Therapy required; a master’s or doctoral degree in Occupational Therapy is preferred.
• Experience in a skilled nursing facility or rehabilitation setting is preferred.
• Proficiency with electronic documentation systems and computer applications.
• Experience with Net Health and PointClickCare is preferred.
• Ability to interpret clinical procedures, policies, and guidelines.
• Strong communication, organizational, problem-solving, and interpersonal skills.
• Ability to work with an interdisciplinary healthcare team and provide resident-centered care.

Responsibilities

• Evaluate residents to determine OT needs per physician orders.
• Develop and update individualized treatment plans to improve ADLs.
• Monitor progress and adjust plans to meet goals.
• Educate residents, families, caregivers on goals and home programs.
• Recommend durable medical equipment and adaptive devices.
• Supervise COTAs, aides, and students.
• Assist nursing with Restorative Aides training.
• Communicate with physicians and staff on progress and discharge.
• Participate in care conferences and rehabilitation meetings.
• Provide in-service education to facility staff.
• Maintain accurate documentation per policy.
• Maintain safe treatment environment and infection control.
• Report equipment issues and ensure safety.
• Participate in quality assurance and program development.
• Maintain confidentiality and professional relationships.
