Roseville Care Center in Roseville, CA is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team. The PT evaluates residents, develops individualized treatment plans, and provides therapy to improve mobility, strength, balance, and independence.

The ideal candidate will have a CA PT license, SNF experience, strong communication, and the ability to work with an interdisciplinary team to help residents achieve their highest level of function.

Qualifications

• Bachelor’s degree in Physical Therapy required; a master’s or doctoral degree in PT preferred.
• California Physical Therapist license in good standing required.
• Experience in a skilled nursing facility or rehabilitation setting preferred.
• Proficiency with computer applications and electronic medical record documentation (Net Health, PointClickCare preferred).

Responsabilités

• Evaluate residents to determine PT needs and rehabilitation potential per physician orders.
• Develop and update individualized treatment plans to improve mobility, strength, balance, endurance, and independence.
• Monitor progress and modify treatment plans to achieve goals.
• Supervise PTAs, aides, and students as applicable and per laws.
• Document accurately in medical records and maintain confidentiality.
• Educate residents and families on goals, home exercise programs, and mobility techniques.
• Coordinate with physicians and care team for discharge planning and progress updates.
• Participate in quality assurance and rehabilitation program development.
