Job summary

Mill Creek Center for Nursing & Rehabilitation in North Carolina seeks a compassionate Occupational Therapist (OT) to provide evaluation-based treatment in a long-term care setting. You will develop individualized plans based on physician orders and deliver therapies onsite.

The role involves supervising COTAs and rehab staff, maintaining documentation, and collaborating with an interdisciplinary team to support residents' daily functioning and mobility goals.

Qualifications

• Valid state licensure as a Physical Therapist.
• Prior experience in long-term care is a plus.
• Strong communication and organizational skills.
• Passion for working with geriatric population.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans based on physician orders.
• Deliver occupational therapy treatments and adjust plans to achieve functional goals.
• Supervise COTAs, rehab techs, and therapy students as required.
• Maintain accurate timely documentation per regulatory standards.
• Collaborate with nurses, physicians, and physical therapists.
• Educate residents, families, and staff on therapy techniques and home exercise programs.
• Participate in discharge planning and facility quality improvement initiatives.
